In consideration of being allowed to participate in Forms Academy programs, I acknowledge that the risk of injury from soccer training and competition is significant, including the potential for permanent paralysis and death, and while rules, equipment, and personal discipline may reduce this risk, the risk of serious injury exists. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, and assume full responsibility for my player's participation. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS Forms Academy LLC, its officers, officials, agents, employees, coaches, volunteers, sponsors, and owners and lessors of premises used ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I understand the signs and dangers of concussion, that my player will be removed from activity if a concussion is suspected, and that return to play requires clearance consistent with applicable concussion protocols.
If my player is injured or ill and I cannot be reached, I authorize Forms Academy staff to obtain emergency medical treatment, including ambulance transport and treatment by licensed medical professionals. All medical expenses remain my responsibility.
I consent to photographs and video of my player taken during Forms Academy activities being used in Forms Academy materials, including its website and social media, without compensation.
Guest play: my player may not guest-play for any outside club, team, or organization without prior written permission from the Academy Director. External training, including camps, clinics, and private sessions outside Forms Academy, likewise requires prior written permission from the Academy Director. I agree to the Forms Academy parent code of conduct, including positive sideline behavior, respect for all game officials, and the 24-hour rule for game-related concerns. Violations may result in suspension or termination of membership.
Tuition bills monthly (or annually if selected) to my card on file, with a one-time $99 registration fee at signup unless waived by code. Each Academy family contributes a $250 club fee charged October 1 and April 1. I may cancel with 30 days notice; the current billing period is not refunded. The 30-day money-back guarantee applies to my first month as described on this page.

Older LeagueApps rates end with LeagueApps. Every family moves to the published 2026-27 Academy plans. The 10 percent second-child discount is applied automatically at setup. Something looks wrong? Tell us before you continue.
After you finish: the $250 facilities and equipment fee is charged on 1 October, your first tuition payment on 12 October, then the 12th monthly. A one-time invoice for fall league registration and referee fees comes separately later this fall, with the amount and due date in that email.
Why the Master? One login for your family: rosters, lineups, schedule, RSVP, messages, billing and receipts. Coaches build game-day rosters in it, and league schedules feed into it automatically. LeagueApps only ever handled payments and could not talk to any of that.
Will I pay more? Everyone moves to the published 2026-27 plans: Academy Monthly $175 ($180.54 by card), Academy Entry $125 ($128.96 by card), with annual paid-in-full options and a 10 percent second-child discount. Card processing is passed through, slightly less than LeagueApps charged.
What about the September charge I just paid? That should be your last LeagueApps installment. Your first Master charge is 12 October. If LeagueApps charges you again after you have set up here, we credit it, so you will not pay twice.
Can I pay by bank transfer or check? Card is the standard for monthly tuition. If you need another arrangement, email admin@formsacademy.com before the deadline.
Is the facilities fee new? No. It has always been $250 per family on 1 October and 1 April. It now runs through the Master.
We are a Residency family too. Academy tuition and Residency tuition are separate programs and are billed separately. Both run through the Master.
What if I miss the deadline? Your LeagueApps plan keeps running and we send a second link. From 1 November a player without a Master plan cannot be placed on a squad.
