Club fee · $250 twice a year

Alongside tuition, each Academy family contributes a $250 club fee on October 1 and April 1. It funds what tuition alone does not: field and training-space costs, facility rental and lighting, goals and training equipment, field maintenance and storage, insurance, and game-day operations. A reminder arrives one week before each charge.

1. Release of Liability & Assumption of Risk

In consideration of being allowed to participate in Forms Academy programs, I acknowledge that the risk of injury from soccer training and competition is significant, including the potential for permanent paralysis and death, and while rules, equipment, and personal discipline may reduce this risk, the risk of serious injury exists. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, and assume full responsibility for my player's participation. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS Forms Academy LLC, its officers, officials, agents, employees, coaches, volunteers, sponsors, and owners and lessors of premises used ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

2. Concussion Awareness

I understand the signs and dangers of concussion, that my player will be removed from activity if a concussion is suspected, and that return to play requires clearance consistent with applicable concussion protocols.

3. Emergency Medical Authorization

If my player is injured or ill and I cannot be reached, I authorize Forms Academy staff to obtain emergency medical treatment, including ambulance transport and treatment by licensed medical professionals. All medical expenses remain my responsibility.

4. Photo & Video Release

I consent to photographs and video of my player taken during Forms Academy activities being used in Forms Academy materials, including its website and social media, without compensation.

5. Club Rules

Guest play: my player may not guest-play for any outside club, team, or organization without prior written permission from the Academy Director. External training, including camps, clinics, and private sessions outside Forms Academy, likewise requires prior written permission from the Academy Director. I agree to the Forms Academy parent code of conduct, including positive sideline behavior, respect for all game officials, and the 24-hour rule for game-related concerns. Violations may result in suspension or termination of membership.

6. Billing Terms

Tuition bills monthly (or annually if selected) to my card on file, with a one-time $99 registration fee at signup unless waived by code. Each Academy family contributes a $250 club fee charged October 1 and April 1. I may cancel with 30 days notice; the current billing period is not refunded. The 30-day money-back guarantee applies to my first month as described on this page.

I understand the $250 biannual club fee (October 1 and April 1, per family) is part of Academy membership and will be charged to my card on file. I understand there is no contract and no cancellation fee. Tuition is billed monthly and I can cancel anytime with 30 days notice. I acknowledge the liability waiver and consent to participation in training activities. I acknowledge the concussion protocol, authorize emergency medical treatment for my player if I cannot be reached, and consent to photo/video of my player in Forms Academy materials. I agree to the club rules, including that guest play and external training require prior written permission from the Academy Director.
